4. 永远不要修改已接受记录中的决策；另写一条记录取代它，并在两条记录之间建立双向链接。
5. 在实现该决策的代码改动所属的同一个 pull request 中审查记录。

## 预期结果
新加入的人能按顺序阅读记录，重建系统为何形成当前样貌的来龙去脉。推翻旧决策会体现为新记录，而不是悄悄改写旧内容。

## 局限与方法依据
记录描述的是决策，而非设计；图表和规格说明放在其他地方。过多琐碎记录会掩盖重要决策，因此应约定什么程度的重要性值得记录。格式遵循所引来源；价值取决于执行纪律，而非工具。
