Conditions

Record the environment, intended outcome and reproducible inputs without secrets or private logs.

Observation

Describe what actually happened and the minimal evidence.

Hypotheses

List plausible explanations as hypotheses.

Checks

For each check, state the predicted result, observed result and remaining uncertainty.

Limits

Do not generalize a single incident beyond its documented conditions.
