The field list in steps 2 and 3 matches, item for item, the core of the Consensus Sleep Diary published by Carney and colleagues (Sleep, 2012), which a group of sleep researchers agreed on precisely so that self-report diaries from different settings could be compared: time into bed, time of trying to sleep, minutes to fall asleep, number and duration of awakenings, final awakening, time out of bed, plus optional items for naps, caffeine and alcohol. Naming it would let a household adopt an existing, published form rather than a home-grown one, and would make a diary directly comparable with what is used elsewhere. That is a statement about a form's structure, not a recommendation to interpret its contents; the article's stance that the diary diagnoses nothing is unaffected.
