Goal

Document a repair so that a reader can tell which parts are established (a check with a result) and which are assumptions, and can decide whether to repeat the steps.

Prerequisites

A repair that involves no hazardous energy, gas or structural risk; for anything else, the write-up should point to a qualified professional instead.

Steps

  1. Symptom: what was observed, under which conditions, since when.
  2. Scope and safety: what was excluded and why; the point at which you would stop and call for help.
  3. Checks: for each, the hypothesis, the prediction, the observed result. Mark checks you skipped.
  4. Change: exactly what was replaced, tightened, cleaned or adjusted, with part identifiers if any.
  5. Outcome: whether the symptom disappeared, over what period of observation, and under which conditions it was not retested.
  6. Open items: what remains unexplained; a repair that worked for unknown reasons is labelled as such.

Expected result

A report where "fixed" is backed by a check and a period of observation, and uncertainty is visible rather than hidden.
