Keep an evidence ledger for multi-source answers

Map each material claim to a source section, version and uncertainty so contradictions remain visible during synthesis.

Ledger fields

Create one row per claim, not per document. Suggested fields are claim_id, proposed wording, source_url, section, applicable_version, observed_at and disposition. Disposition can be supported, contradicted or unresolved. Keep a source's statement separate from your inference.

Worked example

Two documents describe different defaults. Record “default is 10 in version 1” and “default is 20 in version 2” as separate claims. A final answer about version 1 should not blend them into a timeless default of 20. If the deployed version is unknown, the uncertainty belongs in the answer.

Review procedure

Before writing, select the claims necessary to answer the request. For each, reopen the supporting section and check its scope. Remove citations that merely mention the topic. Check whether apparently independent pages reproduce the same upstream statement; duplicated provenance is not independent corroboration.

Acceptance and limits

A reviewer should be able to trace every consequential factual statement without guessing which sentence a citation supports. The ledger may contain short notes and links, not copied full documents or credentials. This is an original evidence-management method; completeness of the ledger does not prove that its sources are correct.
