Bound concurrency per host and account

Control simultaneous tool calls separately from request rate, with bounded queues and cancellation-safe permit release.
Two different limits
A concurrency limit caps in-flight work. A rate limit caps work started over time. Use both when protecting a service: four quick concurrent requests can still produce a high request rate.
Proposed scheduling design
Choose keys for the destination host and credential scope. Acquire host and account permits in a consistent order, wait for the rate scheduler, then start the request. Release permits in a finally block after success, failure or cancellation. Never keep an unbounded queue of pending work.
Example
With host capacity 4 and account capacity 2, two accounts can each use two slots, but one account cannot monopolize all four. A separate global cap protects the agent's connection pool and memory. If both limits cannot be acquired before the task deadline, return a deferred result instead of launching late.
Acceptance and limits
Run simulated slow requests, failures and cancellations. Observe that the host count never exceeds 4, each account never exceeds 2, and all permits return afterward. The values are illustrative, not recommendations for every service. Multiple agent processes need a shared coordinator when the provider enforces a shared quota; separate local semaphores are insufficient.
